Fidelis Care Maternal Health Grant Program 2025 (up to $20,000)

Applications are now open for the Fidelis Care Maternal Health Grant Program 2025, an important initiative aimed at advancing maternal health across New York State. Through this program, Fidelis Care is offering grants to eligible healthcare providers and community-based organizations that are dedicated to improving the health and well-being of mothers, expectant mothers, and families. The goal of the grant is to support innovative, impactful maternal health and wellness programs that address critical needs, reduce disparities, and ensure better outcomes for underserved communities. By investing in local organizations and care providers, It is reinforcing its commitment to promoting equitable access to quality maternal healthcare and strengthening community support systems statewide.

This year’s Maternal Health Grant Program is placing a strong emphasis on supporting innovative and impactful strategies that address two critical areas: postpartum care and maternal mental health. As maternal health challenges continue to disproportionately affect high-need and low-income women and families, the program seeks to fund forward-thinking initiatives that can make a meaningful difference in these communities. Applicants are strongly encouraged to ensure that their proposals are closely aligned with these focus areas and demonstrate a clear commitment to improving outcomes during the postpartum period, as well as addressing the emotional and psychological well-being of mothers.

Eligibility for Fidelis Care

• To be eligible, providers or community organizations must serve New York State.
• Applicants must answer all of the questions on the form to be considered.
